11.2 Recognizing Obsessions vs Compulsions: Obsessions are intrusive thoughts, images, or urges that show up without warning, causing stress or fear, while compulsions are behaviors or mental actions you feel forced to do in order to reduce that fear or prevent something bad. You learn to separate the two by asking: “Is this thought unwanted and repetitive?” (obsession) or “Do I feel like I must do this to feel okay?” (compulsion). Sometimes compulsions happen inside the mind—counting, praying, reviewing memories, or trying to “cancel out” a disturbing thought—and are harder to notice. Recognizing the difference helps you understand how OCD grabs control: the brain sends a false alarm, then demands you respond to it with ritual. When you identify obsession and compulsion clearly, you gain power to interrupt the cycle. You stop treating thoughts like commands, and you stop believing rituals are the only way to stay safe.
